By accessing or using the services provided by Empower Performance PLLC, you agree to be bound by these Terms of Service.
If you do not agree to these terms, please do not use our services.
Services Description
Empower Performance provides physical therapy, strength and conditioning performance training, return to sport, and soccer training in individualized and group setting.
Appointment and Cancellation Policy
Appointments can be scheduled online or by phone. We request 24-hour notice for cancellations. Late cancellations or no-shows may be subject to a fee. We reserve the right to cancel or reschedule appointments as needed.
Payment Terms
Payment is due at the time of service unless other arrangements are made. We accept cash, credit cards, check, Zelle. We are a cash-based self-pay service and payments are due at time of service. We are a out of network for physical therapy services only and you may request a super-bill to submit to your own insurance. Empower Performance does not bill insurance directly.
Outstanding balances may be subject to collection procedures
Assumption of Risk
Physical therapy, training, and exercise programs involve inherent risks. By participating in our services, you acknowledge and assume these risks. You should consult with a physician before beginning any exercise program.
Limitation of Liability
Empower Performance PLLC and its staff shall not be liable for any injuries, damages, or losses arising from participation in our programs, except in cases of gross negligence or willful misconduct.
